Our Care Coordination Department can provide case management services that include:

• Completing a health assessment to help us identify your health care needs
• Providing the latest health information - to help you understand your condition and help you stay healthy
• Help you move through the health care system - to make sure you get the right care at the right time in the right place
• Assistance with making appointments to your regular doctor and specialists
• Creating a care plan that will help you discuss your health care needs with your doctor
• Coordinate your transportation needs if you have a medical disability that prevents you from using public transportation
• We can provide the keys to successfully managing your health condition!

Care Coordination Department:

• Qualified health care professionals such as a nurses and social workers

• The program is voluntary and everything is kept completely confidential

• If you choose not to participate, you can stop at any time
