Medicaid

Most Medicaid consumers now have to choose a managed care health plan. Neighborhood Health Providers is the right choice! Medicaid is the major federal/state program that provides health insurance for individuals and their families, particularly children, pregnant women, the elderly and the disabled. Eligible individuals receive a Medicaid benefit card.

Neighborhood Health Providers (NHP) offers most Medicaid benefits. Benefits not covered by NHP are offered through Medicaid Fee-For-Service. Medicaid eligible persons will use their Medicaid benefit card for benefits covered by Fee-For-Service Medicaid (example: pharmacy). The NHP ID card will be used for all covered benefits.

Adults can

Join if they:
• Already receive a Medicaid benefit card (New York Medicaid CHOICE)
• Meet income criteria

NHP Medicaid Members provides the following benefits:

Regular Check-ups with your primary care doctor Vision and hearing services
Baby and child care, plus immunizations Laboratory, x-rays and other medical tests
Hospital and emergency services Special
Dental services Prenatal and maternity care
Specialist visits Hospital services
