Q:
How can I enroll in Neighborhood Health Providers (NHP) Managed Care Program?
A:

You may contact us at the following numbers to set up an appointment with one of our marketing representatives who will assist you with completing an application.

Q:
How do I get help after normal business hours?
A: There is someone to help you at Member Services Monday through Friday, 9:00AM - 5:00PM. If you need to call at other times, an operator will assist you when you call Member Services.
Q:
Do I need a referral to receive routine eye care?
A: You DO NOT need a referral from your Primary Care Provider for eye exam services. You must call NHP's toll free vision care number at 1-800-999-5431 to find the nearest participating eye doctor.
Q:
How do I change my Primary Care Provider (PCP)?
A: If you need to, you can change your PCP in the first 30 days after your first appointment with your PCP. After that, you can change every six months without cause or more often if you have good reason. Please contact member services.
Q:
Does my dentist have to be in the same location as my Primary Care Provider?
A: No. Neighborhood Health Providers has a network of Primary Care Dentists (PCD) in your area to assist you. You may call 1-800-468 9868 for any questions about the dental program or to get a list of Primary Care Dentists convenient to you.
Q:
What do I do if I lose my Neighborhood Health Providers ID card?
A: Please contact Member Services. A new ID card will be sent to you.
Q:
I have recently moved. How can I change my address?
A: Call Member Services at 1-800-826-6240 and your local department of Social Services to advise us of any changes in your life.
Q:
What do I do if I receive a bill?
A: You may have to pay for any service(s) that your PCP does not OK. This includes non-covered services, unauthorized services and/or services provided by providers who are not part of Neighborhood Health Providers. If you have any questions, call Member Services at 1-800-826-6240.
Q:
Do I have the right to a second opinion regarding a course of treatment or surgery?
A: Yes. To request this, your Primary Care Provider must complete a NHP Prior Approval Request Form. The NHP Medical Director will review your request.
Q:
Where can I go for Urgent Care?
A: You can get an appointment for an urgent care visit for same day or next day. If you are home or away, contact your PCP, any time, day or night. If you cannot reach your PCP, call Member Services at 1-800-826-6240. Tell the person who answers what is happening. They will tell you what to do.
Q:
When I get my NHP ID Card can I stop using my Medicaid benefit card?
A: No, you will need to use your Medicaid benefit card to get services that NHP does not cover. These exclusions vary by plan.
