Asthma Program NHP has an asthma case management program that is available to all members who have been diagnosed with asthma.

Our Case Managers are available to:

• Talk to members to review their health history
• Guide the member in obtaining the right care for their condition
• Coordinate physician appointments
• Assist members in gaining access to equipment such as nebulizers and the correct medications.
• Provide information to the member about their conditions and recommended treatments so they or their parent or spouse can make informed decisions regarding their health care treatment.

The Case Managers works with the member and physician's to improve or maintain the members current health status.
