The goals of the NHP case management program:

The goals of the Neighborhood Health Providers case management program are to help the member understand their condition, promote correct eating habits and diet maintenance and assist the member in the prevention of complications from their conditions.

Case Management
NHP has a case management program that is available to all members who have been special health care needs.

Our Case Managers are available to:

  • Discuss difficult cases with physicians
  • Talk to members to review their health history
  • Guide the member in obtaining the right care for their condition
  • Coordinate physician appointments
  • Coordinate care with the treating physician(s)
  • Assist members in gaining access to durable medical equipment, monitoring devices and Rx.
  • Provide information to the member about their conditions and recommended treatments so they or their parent or spouse can make informed decisions regarding their health care treatment.

The Case Managers works with the member and physician's to improve or maintain the members current health status.
