Care Coordination Program
NHP's Care Coordination Program is designed to help our members manage serious or disabling medical conditions such as:
- Asthma
- Cancer
- Diabetes
- Heart problems
- High Risk Pregnancy
- HIV/AIDS
Our Care Coordination Department provides case management services that include:
- Completing a health assessment to help us identify members' health care needs
- Providing the latest health information to help our members understand their condition and stay
healthy
- Helping our members move through the health care system, ensuring that they receive the right care, at the right
time, in the right place
- Assisting members in making appointments with their regular doctor or specialists
- Creating a care plan to help our members discuss their health care needs with you, the provider
- Coordination of transportation needs if members have a medical disability that prevents them from using
public transportation
About our Care Coordination Department:
- Qualified health care
professionals such as nurses and social workers
- The program is voluntary
and everything is kept
completely confidential
- If a member chooses to no longer participate, he or she can stop
at any time
