The goals of the Diabetes case management program:

The goals of the Neighborhood Health Providers diabetes case management program are to help the member understand their condition, promote correct eating habits and diet maintenance and assist the member in the prevention of complications from diabetes.

Diabetes Program
NHP has an diabetes case management program that is available to all members who have been diagnosed with diabetes.

Our Case Managers are available to:

  • Talk to members to review their health history
  • Guide the member in obtaining the right care for their condition
  • Coordinate physician appointments
  • Assist members in gaining access to glucose monitors and the correct medications.
  • Provide information to the member about their conditions and recommended treatments so they or their parent or spouse can make informed decisions regarding their health care treatment.

The Case Managers works with the member and physician's to improve or maintain the members current health status.
