Frequency of periodic exams are at physicianıs discretion for specific diagnostic procedures.

 

 

SCREENING

 

History

 

 

Medical, mental health,

social, and family history

Dietary intake

Physical activity

Tobacco / alcohol / drug use

Sexual practice

Abuse/family dysfunction

 

 

Complete Physical Examination

 

Including:

Height, Weight,

Blood Pressure

Examination for cancer

To include:

Skin, oral cavity,

thyroid, breast, lymph

nodes, testicles,

prostate, pelvic

examination

Digital rectal examination

Auscultation for carotid

Bruits

Fundoscopy

 

 

 

Laboratory / Diagnostic Procedures

 

Including but not limited to:

Non-fasting total blood

Cholesterol

PAP smear annually (after three normal exams, PAP may be done every three years)

Rubella antibodies ­ females VDRL RPR (HRS)

TB skin test (PPD)

 

COUNSELING

 

Diet and Exercise

Injury Prevention

Dental Health

Weight Control

 

 

Tobacco, alcohol, drug use

Breast and testicular

Self exam

 

 

Sexual practice

STD

Unintended pregnancy and contraception options

HIV ­ shared needles

Td booster ­ every 10 years
